How to Prescribe

We offer providers four ways to send prescriptions, with an online portal coming soon.



Escribe

Find us by selecting Chemistry Rx from the dropdown. Choose Miscellaneous as medication. In notes or comments, write the formula you wish to prescribe.



Phone

Call us at 855-790-0100 and a member of our pharmacy staff will fill your prescription.



Email

Email info@chemistryrx.com and one of our pharmacists will fill the prescription.



Fax

Fax your prescription to 267-861-0862. Please let us know if you’re interested in a custom script pad to alleviate any issues with prescribing.
